Job Summary">

We are seeking a skilled and compassionate Community Nurse to join our team at Wrightington Wigan & Leigh Teaching Hospitals NHS Foundation Trust.

The successful candidate will be responsible for providing high-quality care to patients in the community, working closely with multidisciplinary teams to ensure safe and effective care delivery.

This is a full-time role on a 12-month fixed-term basis, covering maternity leave. If the substantive postholder does not return, it may be made permanent.

The salary for this position is approximately £34,000 per annum, depending on experience.

Main Responsibilities

  • Provide exceptional patient care, ensuring high standards of care and adherence to NMC Code of Conduct.
  • Work collaboratively with healthcare professionals to develop and implement personalized care plans.
  • Participate in clinical supervision and audit research to improve care quality and outcomes.
  • Maintain accurate and legible records, adhering to trust policies and procedures.
  • Communicate effectively with patients, families, and colleagues, promoting dignity and respect.

Requirements

  • NMC registration and current practising certificate.
  • Relevant qualifications, such as a degree or diploma in nursing.
  • Experience in frailty care and community nursing settings.
  • Excellent verbal and written communication skills, basic computer literacy, and ability to work independently and as part of a team.
