Job Title: Community Cardiology Care Coordinator

We are looking for an experienced cardiac nurse to join our team as a Community Cardiology Care Coordinator. This is an exciting opportunity to work in a dynamic and rewarding environment, making a real difference in the lives of patients living with heart conditions.

About the Role

As a Community Cardiology Care Coordinator, you will work closely with patients, families, and healthcare professionals to provide exceptional care and support in the community setting. Your expertise will enable you to provide specialist nursing advice, education, and support to patients and their carers, contributing to the development of clinical policies and protocols.

The ideal candidate will have significant post-registration experience in cardiology or cardiothoracic nursing, excellent communication skills, and the ability to work autonomously within local and national guidelines. They will also possess a relevant degree or diploma and be registered with the NMC.

Key Responsibilities
  • Provide specialist nursing advice and education to patients and their carers.
  • Contribute to the development of clinical policies and protocols.
  • Work closely with multidisciplinary teams to ensure seamless patient care.
  • Maintain accurate records and reports.
Person Specification

The successful candidate will have:

  • Significant post-registration experience in cardiology or cardiothoracic nursing.
  • Excellent communication and interpersonal skills.
  • A relevant degree or diploma.
  • NMC registration.

We offer a competitive salary of £43,041 - £56,219 per annum, depending on experience, plus benefits including pension scheme, life assurance, and annual leave.
