You will work closely with our integrated team of health and social care workers, as well as other external health professionals, to maximise the support available to our customers.
We offer a comprehensive induction and training program, as well as ongoing support to ensure your success in this role.
To succeed in this role, you will need:
* To hold a QCF Diploma or NVQ Level 2 in a relevant area, with a willingness to obtain Level 3
* Experience of working in care
* A driving license and use of a personal vehicle for work purposes
* Confidence in travelling around the Essex area
* An enhanced DBS check, which can be arranged through ECL if needed
* Exceptional communication skills
* The ability to build and develop strong relationships with stakeholders
* Solid IT skills
* The ability and confidence to work independently and manage your own time
* A proactive, adaptable, and customer-focused work ethic
