Community Outreach Worker Job Description

Nurseplus is seeking a Community Outreach Worker to join our team.

The successful candidate will have experience working with vulnerable populations, including those with mental health conditions, substance misuse issues, and learning disabilities.

This role involves providing outreach services, developing relationships with community members, and linking them with available resources and support services.

The Community Outreach Worker will work closely with healthcare professionals, social workers, and other agencies to ensure that individuals receive comprehensive care and support.

We estimate the salary for this position to be around £30,000 - £38,000 per annum, depending on experience.

Key Responsibilities:

  • Provide outreach services to vulnerable populations
  • Develop relationships with community members and link them with available resources
  • Work collaboratively with healthcare professionals and other agencies
