Primary Care Physician

We are seeking a highly skilled and motivated General Practitioner to join our dynamic Primary Care Network team. As a key member of our team, you will be responsible for delivering high-quality patient care and contributing to the development of our local services.
The successful candidate will have a strong passion for primary care and a commitment to delivering excellent patient outcomes. You will be expected to work collaboratively with our practices and PCN staff to achieve our goals and objectives.
Key Responsibilities- Conducting surgeries and telephone consultations to address patient queries and concerns
- Visiting patients at home to provide personalized care and support
- Making professional decisions regarding presenting problems and assessing patient needs
- Screening patients for disease risk factors and early signs of illness
- Providing counseling and health education to patients
- Compiling and issuing computer-generated prescriptions and managing medication
- Participating in on-call sessions and home visiting sessions as allocated in the practice rota
- Qualified General Practitioner with a strong understanding of primary care
- Experience of working in a primary care environment and contributing to clinical audit
- Excellent communication and interpersonal skills
- Ability to work autonomously and as part of a team
- Strong problem-solving and analytical skills
- Experience with medicines management and clinical risk management
