Job Description

As a Chemical Dependency Case Manager, your key responsibilities will include:

  • Creating and reviewing recovery care plans to meet the needs of service users.
  • Providing psychosocial and psychological interventions as part of care packages.
  • Conducting risk/need assessments and implementing risk management/care plans and reviews.
  • Collaborating with health and social care professionals to ensure effective care delivery.
