Job Opportunity: Community Care Professional

We are currently seeking a qualified and experienced Community Care Professional to join our Community Admissions Avoidance Therapy Team (CAATT). The successful applicant will be responsible for providing occupational therapy interventions in hospital emergency departments and medical admissions units, with the goal of reducing hospital admissions and promoting community-based care.

The ideal candidate will possess excellent communication skills, be able to work effectively within a multidisciplinary team, and demonstrate a strong commitment to delivering high-quality services. You will have experience working in acute settings and be able to think creatively when developing and implementing therapy plans.

In this role, you will have the opportunity to work closely with patients, families, and healthcare professionals to develop and implement individualised care plans that meet the unique needs of each patient. You will also contribute to the development of policies and procedures aimed at improving the quality and efficiency of care delivered by the CAATT team.
