About the Role

The Cumbria, Northumberland, Tyne and Wear (CNTW) NHS Foundation Trust is seeking a highly skilled Community Practitioner to join our Early Intervention in Psychosis team. This dynamic role offers an excellent opportunity for experienced professionals to make a meaningful difference in the lives of individuals experiencing their first episode of psychosis.

As a Community Care Specialist, you will work closely with a multidisciplinary team to deliver high-quality individualized care, ensuring safety and promoting recovery and wellbeing. Your expertise will be essential in assessing and managing service users with complex needs, developing and implementing evidence-based therapeutic interventions, and collaborating with other healthcare professionals to achieve optimal outcomes.

Key Responsibilities

• Assess and manage service users with complex mental health needs
• Develop and implement evidence-based therapeutic interventions, including Structured Clinical Management, CBT, and Family Interventions
• Collaborate with other healthcare professionals to achieve optimal patient outcomes
• Participate in duty rota and triaging new referrals
• Support health promotion and education initiatives
• Adhere to principles of safeguarding and report incidents as necessary

Required Skills and Qualifications

• Professional qualification and current registration as a First-Level Registered Nurse (RNMH, RMN, RNLD), AHP, Social Worker, or Clinical Psychologist
• Evidence of post-registration training in clinical/psychological interventions/skills relevant to the post
• Proven track record of post-registration clinical experience relevant to the post
• Mentor Training
• Up-to-date knowledge/experience of relevant legislation and policies
• Ability to carry out comprehensive assessments and provide written reports
• Excellent communication and interpersonal skills

Salary Information

We offer a competitive salary commensurate with experience, estimated around £38,000-£45,000 per annum. In addition to a generous salary, we also provide opportunities for professional development and growth within our organization.
