Job Summary:

We are seeking a skilled Mental Health Liaison Nurse to join our team at Service Care Solutions Ltd. As a Mental Health Liaison Nurse, you will work closely with patients, healthcare professionals, and other stakeholders to provide high-quality care and support.

Key Responsibilities:

  • Work as a liaison practitioner within a multidisciplinary team (MDT) to develop positive relationships with patients who have found it challenging to engage with services.
  • Conduct one-to-one work with patients in various settings, including their homes, GP surgeries, and community centers, to develop care plans, safety plans, and risk assessments.
  • Utilize different interventions, such as distress tolerance techniques, anxiety management, and social circumstances support, to help patients manage their mental health.
  • Collaborate with patients to identify and address social issues, such as housing and benefits, that may be impacting their mental well-being.

Requirements:

  • Current registration with the Nursing and Midwifery Council (NMC) as a Mental Health Nurse.
  • Fully enhanced DBS check.
  • Community experience and a strong understanding of mental health principles.

Additional Information:

  • The hourly rate of £27 is paid via umbrella. We also offer an option to accrue a 'holiday pot,' which would result in an hourly rate of £24.30 and a build-up of holiday time.
