About the Position

The successful candidate will be responsible for conducting assessments, developing case plans, and monitoring progress. You will work closely with internal teams to ensure effective service delivery and collaborate with external agencies to access additional resources and support.

The salary for this role is £32,000 - £36,000 per annum, depending on experience. We offer a comprehensive benefits package, including 26 days annual leave, pension scheme, and opportunities for professional development.
