**Job Description**

We are seeking a highly motivated and empathetic individual to join our team as a Permanent Project Worker supporting our Mental Health & Addiction Services at Rosewood Community Wellbeing Service.

The successful candidate will be responsible for delivering high-quality support services to individuals, ensuring their needs are met, and empowering them to achieve their goals.

**Key Responsibilities**

- Provide person-centred support to individuals with mental ill health, intellectual disability, autism, and addictions

- Develop and implement tailored support plans to meet individual needs

- Collaborate with multidisciplinary teams to ensure comprehensive care and support

- Maintain accurate records and reports to track progress and outcomes

- Participate in training and professional development to enhance skills and knowledge

**Benefits**

We offer a competitive salary of £24,000 - £27,500 per annum, plus benefits including:
