Key Responsibilities:

* Provide clinical services to patients, including surgery consultations, telephone consultations, and home visits
* Assess patient health needs and develop care plans in consultation with patients and in line with current practice disease management protocols
* Screen patients for disease risk factors and early signs of illness
* Provide counseling and health education
* Admit or discharge patients to and from the caseload and refer to other care providers as appropriate
* Record clear and contemporaneous consultation notes to agreed standards
* Compile and issue computer-generated acute and repeat prescriptions

Requirements:

* Fully qualified GP with current GMC membership
* Experience of working with EMIS
