Job Title: Advanced Physiotherapist Practitioner

About the Role

We are seeking a highly skilled Advanced Physiotherapist Practitioner to join our team at Primary Care Physio. As a key member of our team, you will be responsible for assessing, diagnosing, and managing patients with musculoskeletal conditions, taking ownership of a complex caseload and developing your full scope of clinical practice.

You will work closely with patients to deliver programmes of supported patient self-management, facilitating behavioural change, optimising physical activity, mobility, and independence. In addition, you will develop relationships and collaborate with healthcare professionals across the Primary Care Network (PCN) to integrate pathways and ensure seamless care delivery.

Your responsibilities will also include implementing effective clinical governance, undertaking regular audit and evaluation, supervision, and training to maintain high-quality patient care.
