Job Summary

Thornton Heath Medical Practice is a leading healthcare provider seeking a skilled Practice Nurse to join our clinical team. As a valued member of our team, you will have the opportunity to deliver high-quality nursing care to our patients and contribute to the success of our practice.

Main Responsibilities

  • Implement health promotion and wellbeing programmes to support patients in maintaining good health and preventing illness.
  • Develop and implement individual care plans for patients with chronic diseases, working closely with other healthcare professionals to ensure comprehensive care.
  • Manage and support patients at risk of developing long-term conditions, providing advice and guidance to promote their health and wellbeing.
  • Provide wound care and management, including dressing changes and wound assessments.
  • Run specialist clinics, such as ear syringing and contraception, to provide patients with access to a range of healthcare services.
  • Collect specimens for pathology and request pathology as necessary to support patient diagnosis and treatment.
  • Provide travel advice and vaccinations to patients, ensuring they have the necessary information and support to stay healthy while traveling.
  • Process and interpret limited test results, such as cervical cytology results, to inform patient care and treatment.
  • Maintain accurate and contemporaneous clinical records, ensuring compliance with current regulations and guidelines.
  • Ensure data is coded appropriately to support patient care and practice targets.
  • Implement an effective call/recall system to ensure patients receive timely reminders and appointments.
  • Implement an effective immunisation programme, working closely with other healthcare professionals to ensure patients receive the necessary vaccinations.
  • Chaperone patients as necessary, providing a safe and supportive environment for patients to receive care.
  • Support the clinical team in dealing with clinical emergencies, working collaboratively to ensure patient safety and well-being.
  • Contribute to practice targets, such as Quality and Outcomes Framework (QOF) and local targets, ensuring compliance with guidance and regulations.
  • Liaise with external services and agencies to ensure vulnerable patients receive the necessary support and care.
