A fantastic opportunity has arisen for a Diabetes Care Technician to join our team. If you have compassion, empathy, and the ability to work autonomously, this is the opportunity for you.
You will be part of a multidisciplinary team and work under the supervision and guidance of the Diabetes Specialist Nurses (DSN). Your primary responsibilities will include supporting insulin treatment, gathering and providing feedback on Blood Glucose Levels, training patients on medication, insulins, and equipment, downloading patient BGLs and information, setting up CGMs for DSNs to review, participating in DESMOND/Patient TDM Education, and collecting initial information calls to provide comprehensive information to DSNs for informed triage.
The role includes promoting a better understanding of Diabetes for patients, their families, and carers, and other agencies involved with the patient through education, support, and counseling.
Main Duties of the Job
To work as part of a team, under the supervision and guidance of the Diabetes Specialist Nurses, in the support and care of people with diabetes, in a primary care, community, and domiciliary setting. To work to empower people with diabetes to manage their own condition more effectively and to ensure that the service meets the needs of all client groups.
To understand and support the needs of carers and liaise with other relevant agencies. To work with members of the multidisciplinary team as and when appropriate. To manage own caseload of patients who require ongoing management and support with complex diabetes needs.
