Diabetes Specialist Nurse

Job Overview
A fantastic opportunity has arisen for a skilled professional to work as part of a multidisciplinary team in providing high-quality care to patients with diabetes. If you have compassion, empathy, and the ability to work autonomously, this role may be suitable for you.
You will work closely with the community diabetes team to prevent admission and provide ongoing care for patients with diabetes. Your responsibilities will include managing a caseload of patients with complex diabetes needs, using evidence-based practice and patient-centered goals to assess, plan, implement, and evaluate interventions.
The role also involves promoting better understanding of diabetes for patients, their families, and carers, as well as other agencies involved with the patient through education, support, and counseling.
Main Duties of the Job
- To act as a key worker as appropriate, coordinating multidisciplinary care
- To provide expert support, advice, and education to raise awareness of the needs of patients with diabetes
- To undertake all aspects of clinical duties as an autonomous practitioner with access to advice and support from senior colleagues
- To work closely with both primary and secondary care to ensure a safe and effective patient pathway is in place for all diabetes patients under the care of the community diabetes team
