Job Overview

A unique opportunity has arisen for a skilled Diabetes Specialist Nurse to work with our Peterborough team on a full-time basis. If you possess compassion, empathy, and the ability to work independently, this role may be the perfect fit for you.

Key Responsibilities
  • To act as a key worker, coordinating multidisciplinary care as needed.
  • To provide expert support, advice, and education to raise awareness of the needs of patients with Diabetes.
  • To undertake all aspects of clinical duties as an autonomous practitioner with access to advice and support from senior colleagues.
  • To work closely with both primary and secondary care to ensure a safe and effective patient pathway is in place for all diabetes patients under the care of the community diabetes team.
