Job Description: Integrated Community Children's Care Manager

We are looking for an exceptional Integrated Community Children's Care Manager to join our team at Hampshire and Isle of Wight Healthcare NHS Foundation Trust. The successful candidate will be responsible for managing the development and delivery of high-quality care services for children and young people with complex healthcare needs.

Main Responsibilities:

  • Lead the planning, coordination, and delivery of care services for children and young people with complex healthcare needs.
  • Work closely with families, carers, and the wider multidisciplinary team to develop and implement effective care plans.
  • Manage a caseload of patients, ensuring high-quality, specialist care is delivered.
  • Collaborate with colleagues to develop and implement best practice guidelines, policies, and procedures.
