About the Role:We are seeking a highly skilled and motivated Practice Nurse to join North Staffordshire Combined Healthcare NHS Trust at Holmcroft Surgery. As a key member of our primary care team, you will be responsible for assessing patient needs and developing personalized care plans.
Main Responsibilities:- Conduct comprehensive health assessments and develop treatment plans
- Administer injections, perform venepuncture, and collect specimens
- Manage nurse-led clinics, including travel and childhood immunizations, diabetic and asthma clinics, and smoking cessation programs
- Collaborate with senior clinicians to improve patient outcomes for long-term conditions
- Provide education and counseling on healthy lifestyle choices and risk factor reduction
- NMC registration
- Experience in relevant areas
This is a part-time position, working 25 hours per week. You will be required to work collaboratively as part of a dynamic team and contribute to the overall success of our practice.
