About the Role

We are seeking a highly skilled and experienced Practice Nurse to join our team at Ashford Medical Partnership. As a Practice Nurse, you will play a vital role in providing high-quality patient care and contributing to the success of our organization.

Key Responsibilities
  • Chronic Disease Management
    • Identify and manage chronic diseases, including diabetes, hypertension, and asthma.
    • Develop and implement individualized management plans in collaboration with patients and other healthcare professionals.
  • Therapeutic Monitoring
    • Monitor patients' responses to treatment and make adjustments as necessary.
    • Recognize and respond to potential complications or adverse reactions.
  • Wound Care and Management
    • Assess and manage wounds, including trauma and complex wounds.
    • Develop and implement wound care plans in collaboration with patients and other healthcare professionals.
  • Patient Health Checks
    • Conduct comprehensive health checks, including vital signs and lifestyle assessments.
    • Identify potential health risks and develop strategies for prevention and management.
  • Risk Assessment
    • Identify and assess potential risks to patients, including domestic violence, child protection, and vulnerable adults.
    • Develop and implement strategies for risk mitigation and management.
  • Health Screening
    • Conduct health screenings, including women's and men's health, sexual health, and older adult health.
    • Develop and implement strategies for health promotion and disease prevention.
Requirements
  • Qualifications
    • Registered General Nurse with current registration with the NMC.
    • Qualifications in postnatal checks, diabetic reviews, smears, coils and ring pessaries, compression bandaging and wound care, Zoladex immunisations, ECG and BP measurements, vascular clinics, menopause, catheter care doppler studies family planning and sexual health.
  • Experience
    • Minimum of 3 years' experience as a Practice Nurse.
    • Evidence of continuing professional development activity.
    • Proven ability to evaluate the safety and effectiveness of their own clinical practice.
