Job Description

We are seeking a highly skilled and compassionate Social Worker to join our team in Wychavon, United Kingdom. As a key member of our Adults Team, you will play a vital role in supporting adults and carers to enhance their wellbeing, achieve greater independence, and utilise community resources effectively.

Key Responsibilities:
  • Work with adults using a strength-based approach to address diverse care needs and manage risks.
  • Meet statutory obligations under the Care Act 2014, Mental Capacity Act 2005, and Mental Health Act 1983.
  • Identify and support at-risk adults and carers, using Making Safeguarding Personal principles.
  • Conduct eligibility assessments under the Care Act (2014) through meaningful conversations.
  • Build strong relationships with agencies and the community for effective signposting.
  • Develop support plans focusing on independence and meeting Care Act (2014) requirements.
  • Gather and analyse data for informed decision-making.
  • Identify potential deprivation of liberty issues and prioritize least restrictive care.
  • Promote child welfare and discuss safeguarding concerns with relevant teams.
  • Participate in reflective practice, supervision, and professional development.
Requirements:
  • Professional qualification in social work.
  • Current registration with Social Work England.
  • Significant experience in social work, including assessment and support planning.
  • Extensive experience in social care with adults, especially older adults, and those with physical disabilities.
  • Demonstrated experience in collaborative working with health and social care agencies.
  • Experience in utilising community resources to create sustainable care packages.
  • Ability to travel throughout the county, to families, young people, internal and external clients and within a timely manner.
