Community Rehabilitation Team Lead

We are looking for a highly motivated and experienced Community Rehabilitation Team Lead to join our team. As a key member of our Community Enhanced Rehabilitation Team, you will be responsible for leading the development and implementation of integrated care pathways, including appropriate service user referrals and models of care.
You will participate in case supervision to monitor named primary nurses' standards of service user care and documentation. This is a full-time permanent position with a competitive salary of £37,338 - £44,962 per annum.
The successful candidate will have a strong understanding of recovery models, mental health acts, effective care coordination, and relevant national policies and guidance. You will also possess leadership and management skills, with experience in mentorship, supervision, and coordination of staff.
