About the RoleThe successful candidate will provide professional nursing care and support to patients, carers, and families within a variety of community settings. This will involve working as part of a multi-disciplinary team to assess individual patient's care needs and escalate concerns to the correct member of the team.

The annual salary for this position is £30,420 to £37,030 (pro rata if part-time), along with a comprehensive benefits package.
