We are seeking a highly skilled and experienced Specialist Anticoagulation Nurse to join our Haematology Services team at Buckinghamshire Healthcare Trust. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality, patient-centered care to individuals requiring anticoagulation management.
Key Responsibilities- Assess patients and their families, interpreting complex problems and providing appropriate interventions.
- Collect, collate, evaluate, and report information, maintaining accurate patient records.
- Involve patients and carers/relatives in the planning and delivery of care.
- Make complex decisions regarding anticoagulant dosage changes.
- Monitor investigation results, initiating further assessment/review of treatment pathways as required.
- Manage patients with complications and queries related to anticoagulation.
- Work collaboratively with other professionals and agencies to ensure patient needs are met.
- Establish and maintain effective communication with patients, carers/relatives, and professionals across health and social services.
- Act in accordance with the NMC Code of Conduct.
- Contribute to research projects in conjunction with the Haematology Team.
- Maintain safe and high standards of nursing care in accordance with hospital policies and procedures and national guidelines.
