Job Overview

We are seeking a motivated and dynamic full-time Band 5 Community Nurse to join our Integrated Care Team in Brentwood and Billericay. As a Community Nurse, you will be responsible for assessing, managing, planning, and delivering care to patients allocated to you.

Main Duties
  • Assess and manage patients' needs, developing and implementing individualized care plans.
  • Work collaboratively with other healthcare professionals, including physiotherapists, occupational therapists, and care coordinators, to provide holistic care.
  • Supervise pre-registration students and band 4 nursing assistants/assistant practitioners.
  • Participate in multidisciplinary team (MDT) working, actively supporting and liaising with other health and social professionals and agencies.
Requirements
  • Registered Nurse qualification (RGN)
  • Good communication skills
  • Experience of MDT working (desirable)
  • Experience working in the community (desirable)

NELFT places a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks, which will include mandatory training, systems training, and the allocation of equipment.
