About the Role

Nottinghamshire Healthcare NHS Foundation Trust is seeking an experienced Band 6 Dementia Nurse to join our Memory Assessment Service (MAS) team. As a key member of our multidisciplinary team, you will provide assessment, diagnosis, and signposting to individuals aged 65 and over referred to the service with cognitive impairment.

Job Responsibilities
  • Screening referrals for appropriateness
  • Undertaking initial assessments and creating treatment plans
  • Liaising with the multidisciplinary team to explore symptoms and causes
  • Referring for additional assessments
  • Signposting for post-diagnostic support

The successful candidate will have a strong understanding of older people's mental health needs and be able to demonstrate clinical competence. A full UK driving licence and vehicle for business use are required for this post. However, reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
