Job Title: Memory Assessment Nurse Specialist
\Location: Nottinghamshire Healthcare NHS Foundation Trust
\This exciting role will be working in the Memory Assessment Service (MAS) providing assessment, potential diagnosis and signposting to other services and support networks for individuals aged 65 and over referred to the service with cognitive impairment.
\We are looking to recruit a Band 6 Memory Assessment Nurse who will work as part of the multi-disciplinary team to screen for appropriate referrals, complete an assessment of clinical presentation and symptoms, share information amongst the team and create appropriate and person-centered care plans.
\The successful candidate will review/accept referrals for individuals over the age of 65 with cognitive impairment, completing assessments to ascertain whether or not a Dementia diagnosis is appropriate.
\Key Responsibilities:

- To screen referrals for appropriateness\
- To undertake initial assessments and create a treatment plan\
- To liaise with the MDT to explore symptoms and causes\
- To refer for additional assessments\
- To signpost for post-diagnostic support\
- To have a current knowledge and understanding of older people's mental health needs and ability to demonstrate clinical competence\
- To evidence substantial and considerable experience in assessing and prescribing for people with dementia\
- To be able to manage change and lead by example\
- To possess leadership skills and development project work\
- To work within the Mental Health Services for Older People directorate\
- To have a full UK driving licence and vehicle for business use\
- To act in line with Nottinghamshire Healthcare NHS Foundation Trust Values: Trust Honesty Respect Compassion Teamwork\
- To demonstrate an understanding of and commitment to Equality, Diversity and Inclusion (EDI) and how it applies to their role\
- To earn an estimated salary of £45,000 - £60,000 per annum.\
