About the Role:
We are seeking a compassionate and dedicated Bank Community Carer/HCA to join our team at The Hospice of St Francis. As a Bank Community Carer/HCA, you will provide high-quality care and support to patients in their own homes, working as part of a team to deliver exceptional patient outcomes.
Key Responsibilities:
- Deliver compassionate and person-centered care to patients in their own homes, ensuring their dignity, comfort, and well-being are maintained at all times.
- Work collaboratively with the multidisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
- Provide emotional support and practical assistance to patients and their families, promoting independence and confidence.
- Contribute to the development and maintenance of a positive and supportive team environment, promoting open communication and collaboration.
- Participate in ongoing education and training to maintain and enhance your skills and knowledge, ensuring you remain up-to-date with best practices and guidelines.
Requirements:
- Care Certificate/NVQ Level 2 qualification or equivalent.
- 3 years' experience in a similar role, with a proven track record of delivering high-quality care and support.
- Current driving license and access to a car.
- Ability to work independently and as part of a team, with excellent communication and interpersonal skills.
- Flexibility to work a variety of shifts, including evenings, weekends, and bank holidays.
