About the Role

We are seeking a dynamic and motivated Mental Health Nurse/Occupational Therapist or Social Worker to join our Crisis Resolution and Home Treatment Team as a Band 6 Mental Health Practitioner.

Key Responsibilities
  • Provide first-line assessments of individuals experiencing acute mental health difficulties and act as gatekeepers to acute admissions.
  • Rapidly assess people with mental health problems who are vulnerable to hospital admission.
  • Provide specialist crisis assessments focusing on 'hospital at home'.
