Job Description: We are seeking an experienced Senior Practitioner/Social Worker to join our newly developed working-age mental health and substance misuse service. This role offers hybrid working, reduced caseloads, and a stable, ongoing contract. You will be responsible for providing tailored care plans and interventions, ensuring individuals receive the support they need to progress on their recovery journey.

Key Responsibilities:
  • Providing tailored care plans and interventions for individuals with mental health issues and substance misuse
  • Working collaboratively with a range of people and organizations to achieve positive outcomes
  • Leading the design and development of new service initiatives to drive improvements in care delivery

Requirements: To succeed in this role, you will need:
  • Experience working with individuals who have mental health issues and substance misuse
  • A sound understanding of relevant legislation, policies, and procedures
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and as part of a team
  • A valid Social Work qualification (DipSW, CQSW, CSS, BA in Social Work or equivalent)
  • Registration with Social Work England
