Clinical Pharmacy Associate

Position Overview
As a Clinical Pharmacy Associate, you will play a vital role in our network of GP Practices, collaborating closely with Practice-based Clinical Pharmacists to facilitate the safe and effective management of medication. Your contributions will be essential in enhancing patient care through optimized medication use and minimizing waste.
Key Responsibilities
- Engage with patients to support their medication management, ensuring clarity and understanding through shared decision-making.
- Conduct medication optimization tasks, including reviewing inhaler techniques and performing medicines reconciliation.
- Assist in the management of medication reviews for new care home patients and coordinate medication transfers between care settings.
- Provide expert advice on public health and social care needs, including lifestyle modifications and addressing local health disparities.
- Participate in initiatives aimed at promoting responsible antibiotic use and reducing unnecessary prescriptions.
- Contribute to the implementation of medicines optimization strategies and patient safety evaluations.

Essential Qualifications
- Registered with the General Pharmaceutical Council (GPhC) as a Pharmacy Technician.
- Completion of a BTEC/NVQ level 3 or equivalent in pharmacy services.
Desirable Skills
- Experience with EMIS and community pharmacy systems.
- Familiarity with medicines reconciliation processes.
- Understanding of quality frameworks and incentive schemes.
