We are seeking a highly skilled and experienced Adult Nurse Manager to join our team. This role offers a challenging opportunity to work as part of a dynamic community care provider, specialising in adult complex care.

The successful candidate will be responsible for delivering tailored care plans specific to individual client needs, effectively managing multidisciplinary teams through strong communication and teamwork, and providing ongoing monitoring, support, and assessment of clients' physical and physiological needs within their caseload.

Key Responsibilities:
• Design and deliver bespoke care packages for adult clients
• Collaborate with community teams, CHC nurses, and CCG commissioners to establish and maintain care plans
• Build and maintain relationships with clients' families, ensuring open communication and empathy
• Develop and implement strategies to enhance the quality of care provided by community teams

Requirements:
• RGN with active NMC PIN
• Up-to-date experience of tracheostomy care and ventilators within the last 2 years (Essential)
• Advanced communication skills, both verbal and written, to liaise effectively with patients, families, and multidisciplinary teams
• Suitable for Band 5 / Band 6 Nurses with evidence of effective leadership and management of HCAs
• Full UK driving licence desirable but not essential
