Clinical Care Coordinator

We are seeking a skilled Clinical Care Coordinator to support our healthcare team. The ideal candidate will have a strong background in nursing and excellent communication skills.
Key Responsibilities:
- Assess and monitor patient health, administering medications and treatments as required.

- Provide education and support to patients and their families.

Requirements:
- Registered Nurse (RN) license in good standing with the relevant nursing board.
- Previous experience in a clinical or hospital setting.
