Palliative Care Nurse

About the Role
We are seeking a highly skilled and compassionate Registered Nurse to join our Hospice in Your Home Team. As a Palliative Care Nurse, you will provide high-quality, patient-centered care to individuals with life-limiting conditions in the comfort of their own homes.
Key Responsibilities
- Deliver palliative and end-of-life care to patients in their own homes, working collaboratively with community nursing and social care teams.
- Support patients and their families with symptom management, emotional support, and practical care.
- Work as part of a multidisciplinary team to provide responsive, mobile care services, undertaking shorter visits to multiple patients throughout the shift.
- Act as the RN Shift Lead as required, providing leadership and guidance to the team.
- Undertake line management responsibility for a group of Palliative Care Assistants and Health Care Assistants.
Requirements
- Registered Nurse with post-registration experience in palliative care.
- Knowledge and skills to deliver generic palliative care in the community.
- Full UK driving license and access to a vehicle.
