Compassionate Community Palliative Care Nurse

About the Role
We are seeking a compassionate and skilled Registered Nurse to join our Hospice in Your Home Team. As a Palliative Care Registered Nurse, you will work with patients and their families in their own homes, providing planned 1:1 care and a roaming service.
Key Responsibilities
- Deliver high-quality, patient-centered care to individuals with life-limiting conditions.
- Work collaboratively with a multi-disciplinary team to provide comprehensive support to patients and their families.
- Develop and maintain strong relationships with patients, families, and healthcare professionals.
- Participate in ongoing education and training to enhance your skills and knowledge in palliative care.

Requirements
- Registered Nurse with post-registration experience in palliative care.
- Full UK driving license and access to a vehicle.
