We are seeking an experienced and skilled Practice Nurse to join our team at Park Lane Practice and Phoenix Surgery. This is a fantastic opportunity to work in a forward-thinking environment that values innovation and patient care.
Salary: £32,000 - £43,667 per annum

As a Practice Nurse, you will play a crucial role in providing high-quality nursing care to our patients. You will be responsible for managing chronic diseases, conducting health promotion activities, and working closely with our multidisciplinary team to ensure seamless patient care.
Key Responsibilities
- To provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF.
- To develop and implement health promotion and well-being programmes.
- To identify, manage, and support patients at risk of developing long-term conditions.
- To maintain accurate clinical records in conjunction with extant legislation.
- To assist GPs with minor surgery when required.
Requirements
- Degree in Nursing or equivalent qualification.
- Post-registration experience in primary care or community nursing.
- Excellent communication and interpersonal skills.
- Able to work independently and as part of a team.
- Familiarity with local and national policies related to primary care.
