The Lakes Medical Centre is seeking an experienced Clinical Pharmacist to join our dynamic team. As a key member of our pharmacy team, you will play a vital role in ensuring the safe and effective use of medicines across our patient population.
Main Responsibilities- Conduct clinical medication reviews to identify opportunities for medication optimisation and reduce inappropriate poly-pharmacy.
- Reconcile medicines following hospital discharge and work with patients and community pharmacists to ensure seamless transitions of care.
- Contribute to the reduction of medicine-related hospital admissions and readmissions by supporting patients to get the best outcomes from their medicines.
- Manage patients and healthcare professionals' medicine queries, providing expert advice and guidance.
- Develop and implement practice formularies to improve the quality, safety, and cost-effectiveness of prescribing.
- Implement drug withdrawals and alerts from the MHRA to improve medicines safety.
- Work with primary care professionals and patients to implement NICE and other evidence-based guidelines.
- Run clinics where medicines are the main interventions, providing expert care and support to patients.
- Contribute to multi-morbidity reviews and multidisciplinary reviews, working collaboratively with other healthcare professionals.
- Provide medicines information and training to practice healthcare professionals and admin staff.
- Act as a source of medicines information for the practice team and patients, providing expert advice on doses, side effects, and alternative treatments.
The Lakes Medical Centre is a forward-thinking training practice, committed to providing high-quality patient care to our 10,800+ patients. Our premises house Community mental health and Social prescribing clinics, and we are proud to have trained GP trainees, Advanced Nurse Practitioners, and Foundation year doctors.
We are actively involved in our local PCN and have additional staff, including PCN pharmacists and pharmacy technicians, who make up our medicine management team.
Person Specification- Essential qualifications: Completion of an undergraduate degree in Pharmacy, and registration with the General Pharmaceutical Council.
- Minimum of 2 years postgraduate experience in pharmacy.
- May hold or be working towards a postgraduate pharmacy qualification.
- Demonstrate self-development through continuous professional development activity.
