Job Description

We are seeking a dedicated and compassionate Senior Support Worker to join our Chorley ABI Service team. The successful candidate will work closely with a service user who is moving out of a hospital setting, providing a bespoke adapted supported living service that enables them to lead a fulfilling and valued life.

Key Responsibilities:

  • Develop and implement individualised support plans to promote the service user's independence and well-being.
  • Provide emotional support and guidance to the service user, empowering them to make informed decisions about their life.
  • Work collaboratively with the service user's family and local community to ensure a seamless transition from hospital to community living.
  • Monitor and report on the service user's progress, identifying areas for improvement and implementing changes as needed.

Requirements:

  • Experience working with individuals with complex needs and supporting them to achieve their goals.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with service users, families, and colleagues.
  • Ability to work independently and as part of a team, with a flexible approach to meet the changing needs of the service user.
