Job Summary: We are seeking a skilled Clinical Case Manager to join our team at HCA Healthcare UK. As a key member of the Case Management Team, you will be responsible for assessing, planning, implementing, and evaluating care options and services that meet patients' health needs.

Key Responsibilities:
  • Act as a liaison between patient, hospital, physician, and third-party payers to optimize patients' community treatment/services.
  • Collaborate with healthcare professionals to develop comprehensive discharge plans that meet patients' unique needs.
  • Provide expert guidance on utilization review and discharge planning best practices.

Requirements:
  • NMC Registered Nurse with substantial postgraduate experience (Adult registered with Paediatric Experience).
  • Experience of utilisation review and extensive discharge planning experience essential.
  • Experience as a Senior Staff Nurse/Band 6 or above essential.
  • Possess excellent communication, organisation, and computer skills.
