An exciting opportunity has arisen for a District Nurse to join the Sudbury Community Health Team. The team is based in the modern Health Centre in Sudbury, a historic market town surrounded by picturesque villages.

The Sudbury Community Health Centre is the base for a wide range of health professionals and agencies, including Adult Social Care, mental health services, and a GP practice. This provides the opportunity to work in a fully integrated way with other healthcare professionals.

As a District Nurse, you will be providing nursing services to patients in the community and have the chance to work in an autonomous manner with the support of the wider multi-disciplinary team within the hospital and across the West Suffolk Alliance.

Main duties of the job:

  • Manage a caseload of complex patients with healthcare needs within a designated population, using specialist knowledge to carry out assessments.
  • Assess, develop, and implement individualized and specialist nursing care programs.
  • Evaluate treatment interventions for the caseload and modify healthcare plans.
  • Encourage patients towards self-care and independence, using a health coaching approach and provide training, education, and advice.
  • Work in partnership with other services, the patient, and their family/carer to prevent inappropriate admission to acute hospital settings.
  • Support discharges into the community.
  • Support patients on the virtual ward and develop the skill set within the community team so staff can respond to a wide range of patient needs.
  • Ensure all aspects of care offered by the team meet the agreed National, Professional, and Local standards.
  • Be responsible for the coordination of the day-to-day planning and delivery of District Nursing services within the team, prioritizing clinical needs and delegating where needed to provide an effective service.
