Key Responsibilities:

  • Support vulnerable individuals with homelessness, social exclusion, and crisis admission to hospital or residential care.
  • Work collaboratively with a small team to achieve shared goals.
  • Conduct thorough client needs and risk assessments for all referrals.
  • Develop and implement individual resettlement support plans for tenants.
  • Ensure clients comply with tenancy agreements and provide guidance on welfare rights, budgeting, employment, education, and training.
  • Liaise with relevant housing agencies to secure suitable move-on accommodation for tenants.
  • Establish and maintain relationships with the local community.
  • Ensure accurate and up-to-date communication systems.
  • Maintain Health and Safety incident and accident records.
  • Adhere to agreed organization policies and procedures.

Requirements:

  • Enhanced DBS Check.
  • Proven track record of providing information, advice, and guidance to clients.
  • Basic IT skills suitable for an office environment.
  • Knowledge of advanced housing management tasks, including rent collection, tenancy agreements, and arrears management.
  • Valid driver's license and access to a vehicle with business insurance.
