About the Role:
\We are seeking a Psychosocial Care Specialist to join our team at the Transformation Unit in Nottingham. The successful candidate will be responsible for providing expert-level psychosocial support to patients with cystic fibrosis and their families.
\As a key member of our multidisciplinary team, you will work closely with healthcare professionals to develop and implement individualized care plans that address the social and emotional needs of patients.
\You will have a strong understanding of the complexities of chronic disease management and be able to provide empathetic and supportive care to patients and their families.
\Responsibilities:
\- Assessing the social and emotional needs of patients
- Developing and implementing individualized care plans
- Collaborating with healthcare professionals to ensure seamless patient care
