Job Overview

We are seeking a skilled and dedicated Health Visitor to join our team at Southern Health NHS Foundation Trust. As a key member of our community health team, you will play a vital role in providing high-quality care and support to patients across Hampshire.

The successful candidate will have a minimum of 6 months paid experience in a Health Visitor role within the last 2 years and be registered with the NMC. They will also possess excellent analytical skills, sound decision-making abilities, and a proactive approach to delivering care.

In this rewarding role, you will have the opportunity to work collaboratively with other healthcare professionals and agencies to provide seamless services for children and families within the community. You will assess child and family needs using appropriate assessment and risk assessment tools, develop outcome-focused care plans, and evaluate care given in partnership with clients.

Key Responsibilities:

  • Manage and deliver care for families, children, and communities working in partnership with team members and service users.
  • Provide a proactive, responsive service based on identified health needs whilst leading the delivery of the universal Healthy Child Programme.
  • Work collaboratively with other healthcare professionals and agencies ensuring a seamless delivery of services for children and families within the community.
  • Assess child and family needs, using the appropriate assessment and risk assessment tools.
  • Develop and implement outcome-focused care plans and evaluate care given in partnership with clients.
