About the Role
We are seeking an experienced and motivated Registered Children's Nurse to join our Community Children's Nursing Team as a Band 6 Community Children's Nurse on a Fixed Term contract to cover sickness for 6 months or longer.
Key Responsibilities
- Provide high-quality nursing care to children with short and long-term conditions within their own homes and wider community.
- Lead by example, demonstrating excellent communication skills and ability to work effectively in a team.
- Develop and implement individualized care plans, working closely with patients, families, and other healthcare professionals.
- Conduct thorough assessments, identify needs, and provide education to patients, families, and healthcare professionals.
- Collaborate with other healthcare teams to ensure seamless care coordination and effective communication.
- Manage and prioritize caseloads, delegating tasks and referrals as necessary to ensure efficient and effective care delivery.
- Participate in quality improvement initiatives, contributing to the development of policies and procedures that enhance patient care and outcomes.
Requirements
- Registered Children's Nurse with a strong background in community nursing and team leadership.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Strong assessment and problem-solving skills, with the ability to develop and implement individualized care plans.
- Ability to work autonomously, prioritizing tasks and managing multiple responsibilities.
