About the Role

We are seeking an experienced and skilled Advanced Nurse Practitioner to join our team at Hartlepool and Stockton Health GP Federation. As an Advanced Nurse Practitioner, you will play a key role in delivering high-quality patient care and contributing to the development of our practice.

Key Responsibilities
  • Deliver advanced autonomous clinical skills and a broad and in-depth theoretical knowledge base to provide high-quality patient care.
  • Manage a clinical caseload dealing with presenting patients' needs in a primary care setting.
  • Lead the implementation of clinical contractual needs of practice, including Quality and Outcome Framework and Enhanced Services.
  • Provide expert professional advice to patients, carers, and colleagues and ensure the maintenance of clinical excellence.
  • Develop new and innovative concepts, models, methods, and practices to deliver new and improved care services to meet the needs of service users.
  • Contribute to the planning and implementation of the teaching for existing practice staff and nurses.
  • Monitor the effectiveness of your own clinical practice through quality assurance strategies, such as the use of audit, peer review, and appraisal.
Requirements
  • Minimum four years experience in primary care.
  • Demonstrate evidence of study at master's level / master's degree.
  • Clinical skills training / qualification and experience.
  • Post Graduate Diploma / Degree (Advanced Practice Qualification).
