We are seeking an experienced and skilled Community Staff Nurse to join our team at Epsom and St Helier University Hospitals NHS Trust. Main Duties
- To work as part of a multi-disciplinary team managing a caseload of patients in the community, identifying and meeting their needs.
- To promote patient independence, improving their physical, psychological, and social wellbeing.
- To collaborate with other teams, colleagues, and agencies to ensure innovative solutions enhance patient experience.
- To undertake comprehensive assessments and care plans, facilitating packages of care in partnership with GPs, patients, and healthcare colleagues.
- To work closely with aligned GP surgery, Clinical Commissioning Group, primary and secondary care professionals, and social care agencies.

Required Skills and Qualifications
- A qualified RGN with current registration with NMC.
- Experience in managing complex patient needs, clinical skills, and ability to work unsupervised or with minimal supervision.
- Excellent communication skills and ability to demonstrate evidence-based clinical practice.
Benefits
We offer a competitive salary, £44,606 - £57,191 per annum, depending on experience, plus benefits including annual leave, pension scheme, and professional development opportunities.
