About the Role

We are seeking a highly skilled and dedicated Adult Social Care Practitioner to join our Gloucester Locality Team. As a key member of our team, you will be responsible for undertaking assessments and support plans with individuals who have physical disabilities or other complex needs.

Key Responsibilities
  • Conduct thorough assessments and develop support plans to promote independence and well-being
  • Work collaboratively with families, carers, and other professionals to achieve positive outcomes
  • Develop and maintain effective relationships with service users, their families, and other stakeholders
  • Stay up-to-date with relevant legislation, policies, and procedures to ensure best practice
  • Participate in regular team meetings and contribute to the development of the team's goals and objectives
Requirements
  • Relevant qualification equivalent to NVQ Level 3 or equivalent Health and Social Care experience
  • Training related to Health and Social Care assessments
  • Experience of managing a caseload, including working with individuals with a range of physical disabilities, medical, mental health difficulties, and learning difficulties
  • Evidence of good interpersonal skills, verbal and written communication, and good IT skills
