The Local Care Organisations ('the LCOs') have been established to bring community health and social care professionals together, working towards a shared vision for better health and care services.

Programme and project management roles play a vital role in delivering transformation within the LCOs. The Project Management Office (PMO) and the Adults Social Care Transformation Team collaborate across the LCOs Transformation Portfolio using a common project methodology and approach to change management.

We are recruiting a Transformation Portfolio Coordinator, a critical role ensuring successful delivery of the LCOs transformation portfolio. The Programme Planner will report to the Head of the PMO and is key to supporting the delivery of business change. Effectively, the role acts as the 'coordination hub' of the transformation portfolio, providing a portfolio-wide view on interdependencies, benefits, risk, change and quality management by offering support and challenge to programme and project managers and producing regular portfolio status reports for management teams.

The Transformation Portfolio Coordinator will be responsible for organising and participating in transformation portfolio governance forums, co-developing and deploying effective programme and project management tools, and helping ensure business change is well-planned and delivered in line with the LCOs project methodology.
