**About the Role:**

We are seeking a highly skilled Caring Adult Services Coordinator to join our Adults Social Care team at Think Workforce.

The primary responsibility will be to work on the discharge to practice model, coordinating services for patients prior to discharge from hospital. This involves arranging services to meet the assessed needs of patients and ensuring smooth transitions for individuals.

**Key Responsibilities:**

  • Coordinating patient care plans and reviews to ensure timely discharge.
  • Collaborating with healthcare professionals and stakeholders to facilitate seamless transitions.
  • Undertaking assessments as required to identify individual needs.

**Requirements:**

  • Holds current HCPC registration.
  • Has 3 years of relevant experience, including at least 1 year in adult social care.
  • Eligible to work in the UK.
  • Possesses Enhanced DBS on the Update Service.

**Additional Information:**

This is a temporary position based in Ilford, Greater London, with a minimum of 36 hours per week. The hourly rate for this role is £22.47 (PAYE) or £28.33 (Ltd).
