About the Role

We are seeking a highly skilled and compassionate Mental Health Practitioner to join our Community Mental Health HUB team. As a Duty Mental Health Practitioner, you will play a vital role in providing timely support and assistance to clients who are struggling with their mental health and experiencing emotional distress.

Key Responsibilities

  • Conduct brief assessments to understand the client's current mental health status, risks, and determine appropriate interventions.
  • Offer emotional support, reassurance, and connect clients with additional resources, services, or support groups for ongoing care and assistance.
  • Provide psychoeducation, collaborative problem-solving, and home visits as needed.
  • Work closely with the multidisciplinary team to assess need, plan, implement, and evaluate treatment plans.
  • Monitor and evaluate the effectiveness of interventions and maintain up-to-date written and electronic records.

Person Specification

  • Professional qualification in mental health nursing, social work, or occupational therapy.
  • Demonstrable post-basic experience in mental health, with a sound knowledge base and excellent communication skills.
  • Ability to prioritize and plan workload, with a commitment to equality, diversity, and human rights.
